  • This model is a GIS-based neighborhood segmentation analysis in which we use standard public health data to identify “at risk” neighborhoods where families have the greatest need for information, education, and intervention. As a result, we can isolate specific areas where public health agencies should concentrate their efforts to locate and serve families.  The process for generating an NPM analysis for your jurisdiction includes:

    Identify the specific health risks you want to address in the community;

    Collect sample data on individuals exposed to specific health risks;   
    Geo-code sample data to find communities with highest exposure to health risks; 
    Develop descriptive portrait of audience for outreach; and
    Use descriptive portrait to develop targeted outreach activities.
